Starting June 1, 2024, we will be implementing a new fee structure for completing various forms. Below is a breakdown of the charges:
- $5 per form: If completed at the time of your Wellness appointment or if the request is made to be completed within 3-4 days.
- $15 per form: If requested to be completed by the end of the next business day.
- $5 per form: If we need to provide or print the form for completion.
- $10 or $20: For letters requested for medical or legal reasons, including FMLA forms.
- $5/form if HDP provides the form or we need to print the form to be completed
This includes, but is not limited to: daycare forms, camp forms and sports/IHSAA forms. Please present any form we need to complete at check-in.
